OC Health Advisory Group to conduct community Health Needs Assessment
Community Health Survey Distribution
The Ocean County Health Advisory Group comprised of the Ocean County Health Department, the Long Beach Island Health Department, the four acute care hospitals in the county: Community Medical Center, Kimball Medical Center, Ocean Medical Center and the Southern Ocean Medical Center, plus the two Healthcare Centers in the county: Center for Health Education Medicine and Dentistry (CHEMED) and Ocean Health Initiatives (OHI) have all joined together to conduct a Countywide Community Health Needs Assessment in 2012. This joint effort will assist in better identifying and coordinating healthcare services amongst the various healthcare agencies in the county.
The Community Health Survey is part of the countywide community health needs assessment process and will provide insight in what you as a county resident feels are the pressing healthcare needs in the county. Your participation is crucial in helping to identify what are the most important health issues that impact the health and well being of the residents in Ocean County.
All responses are anonymous and will be part of our final Ocean County Community Health Improvement Plan.
The survey will be available from June 1 – July 30, 2012. Hard copies will also be available at various sites throughout Ocean County.
You can access the survey electronically in English at:
www.surveymonkey.com/s/OceanCountyCommunityHealthSurvey2012.
Alternatively, in Spanish at:
www.surveymonkey.com/s/OceanCountySpanishCommunity HealthSurvey2012
The Ocean County Health Department has also put the survey on its website in both English and Spanish, which can be accessed by visiting http://www.ochd.org/
If you have any questions concerning this survey or require assistance, please contact Brunilda Price at 732-341-9700 ext. 7228 or Mary Gibson at ext. 7280.
