As Ocean County’s designated Care management Organization (CMO) all referrals must be channeled through the Contract Systems Administrator – PerformCare. To make a referral, a youth must be between the ages of 5 and 18; have a primary mental health diagnosis, and is involved with two or more child serving organizations (that includes school).
